Medical vs. Recreational Cannabis
In Germany, cannabis policy is bifurcated. The KCanG governs recreational personal use for adults (home growing, 25g possession limits). The MedCanG governs medical cannabis prescribed by a physician to treat specific symptoms and illnesses.

Following the removal of cannabis from the Narcotics Act (BtMG), any licensed physician in Germany (excluding veterinarians and dentists) can now prescribe medical cannabis using a standard prescription form (E-Rezept or private paper prescription) rather than the highly restricted Narcotics Prescription.
Pharmacies (Apotheken)
Medical cannabis is dispensed exclusively through licensed pharmacies. Many local pharmacies in Hamburg process these prescriptions, though specialised mail-order pharmacies often stock a wider variety of strains and extracts. The pharmacy guarantees pharmaceutical quality.
Health Insurance
Public health insurance (GKV) covers the cost only under strict conditions (e.g., severe illness where standard therapies have failed). This requires prior approval from the insurer.
Many patients operate as private payers (Selbstzahler), covering the cost of the doctor's consultation and the pharmacy medication out-of-pocket.
Medical Cannabis and Driving
The strict 3.5 ng/ml THC blood serum limit that applies to recreational users does not automatically penalize legitimate medical cannabis patients in Germany.
The Patient Privilege: Patients are legally allowed to participate in road traffic (drive a car) while under the influence of their medication, provided that:
- They are taking the cannabis strictly according to the doctor's prescribed dosage.
- They are adapted to the medication (meaning no acute impairment or delayed reactions).
- They do not mix it with alcohol or other illicit drugs.
Travelling Abroad from Hamburg
You cannot simply fly out of Hamburg Airport or cross into Denmark with medical cannabis using just your prescription receipt. Exporting medical cannabis requires strict administrative compliance.
- Schengen Area: You must carry a completed "Article 75 Schengen Certificate" signed by your prescribing doctor and officially authenticated by the competent health authority in Hamburg (Gesundheitsamt).
- Outside Schengen: You must comply with the international narcotics treaties and the specific laws of the destination country. Many countries completely ban the import of medical cannabis, even with German documentation.
- Cruises: Cruise operators often explicitly ban medical cannabis regardless of your legal paperwork.
